D1 Form Requirement

Modified on Wed, Jan 29 at 3:23 PM

In the new system. A D1 Form must be filled out for every transaction that appears under OEM Transactions - No Authorization Documents on the SDRM 2.0 dashboard.

If you do not have information for an OEM transaction, like in cases where no work was performed, you are asked to use your information and upload a document under Authorization Documentation stating the reason for the transaction.

If the newly completed D1 does not automatically associate to the OEM transaction you are asked to manually resolve it. To manually associate a D-1 Form to an OEM Transaction, please follow the steps below.

  1. On your dashboard, identify the OEM Transaction lacking a matching D-1 Authorization Form
  2. Click the yellow Resolve button
  3. Under D-1 Authorization Documents, click Select for the accurate D-1 Authorization Document
  4. Confirm that the accurate D-1 Authorization Document's Select button has changed to Selected
  5. Provide a detailed description of the discrepancy in the textbox
  6. Click Save

In the SDRM system, a D1 Form must be completed for every transaction that appears under OEM Transactions. Having an unresolved OEM transaction for more than 5 days will lock your account, preventing you from completing additional transactions. These situations underscore NASTF policy to gather all customer documentation before requesting data.

To fill out a D1 form, log in to the SDRM portal and go to the D1 Form link and select the appropriate D1 for the job you are performing.
